Housing stabilization

Our Mission

Oasis Pathways helps Minnesotans achieve stable, sustainable housing through Housing Stabilization Services (HSS). Guided by compassion, integrity, and community, we provide personalized support to overcome housing challenges, prevent homelessness, and build lasting stability. With local partnerships and tailored assistance, we’re here to help every step of the way.

Eligible Members

  • Be on Medical Assistance (MA)
  • Be 18 years old or older
  • Have a documented disability or disabling condition, defined as one of the following:
  • A person who is aged, blind or has a disability as described under Title II of the Social Security Act.
  • A person with an injury or illness that is expected to cause extended or long-term incapacitation.
  • A person with a developmental disability (or related condition) or mental illness.

What is housing Stabilization ?


Minnesota Department of Human Services has launched Housing Stabilization Services, a brand-new housing program for all qualified Minnesotans. The service is meant to provide people with disabilities with the person-centered support they need to succeed in their housing pursuits, whether that means relocating across the street, across the state, or even seeking resources to help prevent homelessness.

What's covered under housing stabilization services ?


- Housing consultation helps you plan for housing based on your wants and needs.
- Housing transition helps you plan for, find and move to housing of your own in the community.
- Housing sustaining pays for supports and services to help you keep your housing in the community.

Do I need a waiver to access housing stabilization services?


No. Housing stabilization services are available to people with disabilities who are eligible for Medical Assistance (MA) and meet other eligibility criteria.
